  1. Assume you are a dental assistant new to your job and while x-raying a young woman’s teeth, you forget to place the requisite lead-lined apron over the patient’s abdomen. What will you do?

  2. Document the incident, and don’t forget again. One such incident should not cause harm, but don’t make forgetting a habit.

  3. Assume you are the office manager for a medical clinic. A patient who sees a gynecologist at the clinic has requested that her medical record be sent to a clinic in another state. She has also received treatment at your employing clinic for mental problems. What will you do?

  4. Be sure you understand exactly which medical records the patient wants forwarded, and obtain her written permission to do so. If she wants the entire record forwarded, you can do so with her written permission. If not, forward just those records she has requested to be forwarded.
