Chapter 16 introduces you to the interactive simulated UB-04 form that was created for use with this text. The simulated UB-04 form is a pdf file that can be used with Adobe Reader to view, create, edit, and print UB-04 claims. You will work with the simulated form in the Computer Exploration exercises at the end of each chapter in Part 2, as well as in Chapter 17 when creating claims for the case studies.

The first part of this chapter, "Introduction to the Simulated Form," shows you the basics of using the form. After downloading the blank form and three sample claims from the text's Online Learning Center, you learn how to open one of the sample claims in Adobe Reader, navigate through the form, and print it. This section should be reviewed before completing the Computer Exploration exercises in Part 2.

The second part of the chapter, "Completing a UB-04 Claim," takes you through the steps of completing a hospital claim using the simulated form. By going through the claim completion steps once, you will be prepared to complete the claims required for the case studies in Chapter 17 on your own.
